Bottom line: Spine surgery deserves a higher due-diligence bar than a tourism package can provide. Start with the diagnosis and imaging, seek a clear explanation of alternatives, verify the surgeon and facility, and understand whether hardware, fusion, rehabilitation, and extended follow-up are part of the quote.

Get the diagnosis and indication in writing

Ask the surgeon to identify the diagnosis, level or levels involved, symptoms the surgery is intended to improve, and the reason surgery is recommended now. If the plan is based on imaging alone without a clear clinical explanation, consider obtaining another opinion.

Know whether the operation includes fusion

A decompression-only procedure and a fusion are not the same operation or recovery. AAOS material on lumbar stenosis notes that fusion adds stabilization with bone graft and often rods and screws. Ask what hardware is planned, why fusion is necessary, and whether hardware costs are included.

Compare surgeon experience with your specific procedure

The relevant question is not simply how many years someone has practiced. Ask how often the surgeon performs the proposed operation, what kinds of patients they typically treat, and at which hospital they operate. Verify authorization to practice through ReTHUS and independently confirm the facility.

Ask about neurologic and emergency backup

Spine surgery can involve nerves, blood vessels, anesthesia, and the possibility of unplanned admission. Ask what monitoring is used when appropriate, who is available if a neurologic issue occurs, and how the facility handles complications.

Understand the recovery and walking plan

AAOS guidance notes that patients are often encouraged to begin walking early after lumbar procedures, while recovery varies substantially by the operation. Ask when you are expected to sit, walk, climb stairs, shower, and travel, and whether a brace is part of your plan.

Do not make the return flight the clinical deadline

Long-distance travel after surgery raises additional clot and mobility questions. Keep the return flexible, discuss sitting tolerance and medication with the surgeon, and know the urgent symptoms that should delay travel and trigger evaluation.
